I installed a few small updates last night but there wasn’t anything new related to this issue directly. It’s always the first job, though. I’ll have some time today to look into the video upload specifically.

Found the problem. The API the video upload component uses has been updated and the old version has now been depracated. Authors of these sorts of add-ins needed to refactor some of their code. They were given plenty of notice this was coming but it appears the author of this particular component has gone AWOL from the Discourse community.
The source code is available so I’ll try fixing it myself. Not sure how long it’ll take me so I’ll remove the feature from the forum for now and let people know when it’s back or I have an alternative solution.
